Description

GH22-2211: supporting the implementation of sustainable, comprehensive, high quality HIV prevention and treatment programs in the central region of the republic of kenya under the president?s emergency plan for aids relief (pepfar) - lvct health, in partnership with elizabeth glaser pediatric aids foundation (egpaf) and kenya medical training college (kmtc), is pleased to submit the dhibiti project in response to CDC-RFA-GH22-2211 supporting the implementation of sustainable, comprehensive, high quality HIV prevention and treatment programs in the central region of the republic of kenya under the president's emergency plan for aids relief (pepfar). We have aptly named our project dhibiti (kiswahili for taking control of the HIV epidemic by the county governments) in line with sustainable epidemic goal of the nofo. Dhibiti project will build on the achievements made through the pepfar/CDC kenya work in central kenya to strengthen the county governments' ability to manage, sustain, and transition financing of HIV services in order to accelerate progress towards sustainable epidemic control and the 95:95:95 targets. Dhibiti will collaborate with the counties of kiambu, murang'a, nyeri and kirinyaga, as well as community organizations, to maximize organizational and leadership capacity for direct oversight and management of HIV service delivery in the counties and implement high quality, evidence-informed HIV prevention, case finding and clinical care services along the HIV /TB clinical cascade. In order to close existing gaps and accelerate progress towards sustainable epidemic control, dhibiti proposes to scale up targeted, innovative strategies, differentiated service delivery (DSD) models and differentiated support packages across the four counties. Dhibiti project will build on the consortium's team past achievements to scale up quality HIV prevention services for KPS, PP, agyw and GP; targeted and cost effective HIV case-finding, provide timely linkage to care and art initiation, retention into care, viral suppression, and quality improvement (QI). Key features of dhibiti's approach include supporting human resources for health and enhancing data collection, use and reporting at all levels of the health system while employing electronic data management systems. The dhibiti consortium comprise proven pepfar implementing partners with a current portfolio of pepfar awards, and additional awards through other bilateral, multilateral, and private donors. The dhibiti team offers technical expertise in service delivery and capacity building through technical assistance (TA) and direct implementation. The dhibiti project will also implement capacity building and system strengthening activities of county governments for increased management, leadership, and financing of HIV services. The project will also increase HIV/TB case identification; HIV prevention focusing on key and priority populations, linkage and retention on antiretroviral therapy (art); laboratory quality management systems; institutionalization of quality improvement (QI) strategies; enhanced data systems for monitoring and ensuring program quality. The outcomes of dhibiti project will include; strengthened county-led health systems as evidenced by increased resource allocation, enhanced leadership and implementation of the HIV program by the chmts, improved coverage and quality of HIV services for all populations, resulting in reduced HIV incidence and improved health outcomes, reduced morbidity and mortality among persons living with HIV (plhiv) including those with tuberculosis (TB), and elimination of mother to child transmission of HIV. The total federal funding request for the project is $ 15,000,000, for project year 1.
