Description
Michigan STI surveillance network - in 2023, michigan's combined preliminary gonorrhea and syphilis case count was approximately 18,320 total cases, with a 10.1% average annual increase in reported gonorrhea cases and 13.6% average annual increase in reported primary and secondary syphilis from 2014-2023. The michigan department of health and human services, bureau of HIV and STI programs (mdhhs/bhsp) regularly analyzes and evaluates the disproportionate burden of sexually transmitted infections (stis) experienced by racial, ethnic and gender/sexual minority populations across the state. The health disparities identified influence approaches to promote and allocate STI services throughout the state of michigan. Mdhhs intends to collect and explore additional data and analyses to foster timely data-driven decisions. With this grant, michigan will: • increase the completeness of STI data for gonorrhea and syphilis and decrease data discrepancies through enhanced STI surveillance activities, such as provider investigations and patient interviews. • collect enhanced data elements, including additional demographic, socioeconomic, and behavioral information on patients with stis to better characterize the extent of health disparities in michigan. • analyze, evaluate, and report adverse health outcomes, manifestations, and changing risk patterns, including the detection of emergent, RE-emergent or novel infections and sequelae associated with STI diagnoses. • link STI investigations with michigan's HIV surveillance records to identify HIV/STI CO-infections and CO-diagnoses, allowing mdhhs to characterize the HIV care outcomes of coinfected people and inform local and national STI/HIV control and prevention efforts, including targeted prevention strategies for STI/HIV programs, as they highlight a priority population of people who are not adequately engaged in HIV care. These reports will also present an opportunity to intervene in multiple disease transmissions utilizing a syndemic approach. • update and standardize quality assurance (QA) reporting and use reports to improve the timeliness and quality of STI data. • implement surveillance data system enhancements to support the collection, automation, and efficiency of data and data reporting. Mdhhs/bhsp is applying for these funds to increase the collection and completeness of STI and HIV data by implementing enhanced case based STI surveillance and updates to michigan's surveillance data system. By strengthening data completeness and the efficiencies within the surveillance system, michigan's STI programs will improve data dissemination and usage, direct retention in care efforts via HIV/STI matching, better understand health disparities among population subgroups, and ultimately inform local and national efforts to promote an equitable level of health to disproportionately burdened individuals. Michigan has the capacity and experience to successfully carry out and meet all components and activities of the ssun grant.
