DESCRIPTION:THIS AGREEMENT FUNDS THE CHEROKEE COUNTY HEALTH DEPARTMENT TO CONDUCT HEALTH EDUCATION ACTIVITIES IN SUPPORT OF THE ONGOING REMEDIAL ACTION AT THE CHEROKEE COUNTY SUPERFUND SITE, WHICH IS LISTED ON THE NATIONAL PRIORITIES LIST (NPL) OF THE NATIONAL OIL AND HAZARDOUS SUBSTANCES CONTINGENCY PLAN. THE CHEROKEE COUNTY HEALTH DEPARTMENT WILL PROVIDE HEALTH EDUCATION TO RESIDENTS ABOUT THE HAZARDS ASSOCIATED WITH MINE WASTE, WAYS TO REDUCE EXPOSURE TO LEAD, AND WAYS TO REDUCE BLOOD LEAD LEVELS IN CHILDREN. THE CHEROKEE COUNTY HEALTH DEPARTMENT WILL ASSIST IN OBTAINING ACCESS FOR INITIAL SOIL SAMPLING AND PROPERTIES THAT QUALIFY FOR REMEDIATION. PLEASE NOTE THAT UNDER THIS COOPERATIVE AGREEMENT, THE CHEROKEE COUNTY HEALTH DEPARTMENT IS NOT BEING PAID OR COMPENSATED TO DRAW BLOOD FOR THE LEAD SCREENING BUT ENCOURAGING TESTING THROUGH EDUCATION AND OUTREACH AND WILL RECORD AND REPORT BLOOD LEAD DATA FROM SCREENING FOR CHILDREN BETWEEN 6-72 MONTHS OF AGE RESIDING IN CHEROKEE COUNTY-GALENA AND BAXTER SPRINGS TO THE EPA. ACTIVITIES:TASK 1 - HEALTH EDUCATION, ACTIVITY 1 -HEALTH EDUCATION PROVIDE HEALTH EDUCATION TO FAMILIES OF CHILDREN WITH AN ELEVATED BLOOD LEAD LEVEL (EBL), PARENTS WITH CHILDREN WHO LIVE IN OR NEAR SUPERFUND SITES OR RESIDE IN PRE-1978 HOUSING, AND PROFESSIONALS IN THE COUNTY SUCH AS PHYSICIANS, REALTORS, CONTRACTORS, DAY CARE, WIC, FARMERS MARKETS AND HEAD START WORKERS. COMMUNITY MONTHLY ACTIVITIES (12 PER YEAR) WILL BE HELD FOCUSING ON LEAD EXPOSURE RISKS. THESE ACTIVITIES MAY INCLUDE, BUT ARE NOT LIMITED TO, PROVIDING HEALTH EDUCATION TO RESIDENTS IN LEAD MINING IMPACTED AREAS, SUCH AS THROUGH PARENT TEACHER CONFERENCES, LOCAL COMMUNITY TOWN FESTIVALS, EDUCATING FAMILIES OF CHILDREN WITH EBL LEVELS THROUGH HEAD START, PARENTS AS TEACHERS, AND WIC SCREENINGS, ALONG WITH PROVIDING AID TO EPA IN EDUCATION EFFORTS RELATED TO ONGOING REMEDIATION ACTIVITIES. OTHER AREAS TO INCLUDE INFORMATION INCLUDE PUBLIC SPEAKING FOR PHILANTHROPIC GROUPS AND ORGANIZATIONS, LOCAL PSA AWARENESS THROUGH NEWS STATIONS AND INFORMATIONAL DISCUSSION IN SCHOOLS OF HEALTHCARE, SUCH AS NURSING PROGRAMS TO REITERATE THE IMPORTANCE OF ASSESSMENT AND TESTING DURING WELL-CHILD SCREENINGS. TASK 1 - HEALTH EDUCATION, ACTIVITY 2 -HEALTH ASSESSMENTS RECORD AND REPORT BLOOD LEAD SCREENING DATA, PROVIDE HEALTH ASSESSMENTS, IN-HOME, COMMUNITY OUTREACH EVENTS AND/OR OFFICE INTERVENTIONS FOR CHILDREN WITH A DETECTION OF LEAD IN THEIR BLOOD. THEY WILL RECORD, REPORT, AND MANAGE DATA FOR BLOOD LEAD SCREENING CONDUCTED IN CHEROKEE COUNTY-GALENA AND BAXTER SPRINGS AND PROVIDE THE RESULTS TO THE EPA ANNUALLY OR UPON REQUEST, TRACKING THE NUMBER OF CHILDREN SAMPLED, RESAMPLED, AND THE BLOOD LEAD TESTING RESULTS AND PROVIDE THE RESULTS TO THE EPA ANNUALLY OR UPON REQUEST, TRACKING THE NUMBER OF CHILDREN SAMPLED, RESAMPLED, AND THE BLOOD LEAD TESTING RESULTS AT THE COUNTY AND CITY LEVEL. TASK 1 - HEALTH EDUCATION, ACTIVITY 3 -INITIATE, CONDUCT AND ATTEND PUBLIC AVAILABILITY MEETINGS ROUTINELY SPONSOR AND ATTEND MEETINGS WHERE THE TOPICS OF HEALTH EDUCATION AND THE HEALTH EDUCATION PROGRAM IS DISCUSSED AND HIGHLIGHTED. THIS MAY ALSO SERVE AS A VALUABLE FORUM TO DISCUSS ONGOING EPA RESPONSE ACTIONS AS APPLICABLE TO INFORM COMMUNITIES OF PROGRESS AND STATUS OF SUCH ACTION. SUCH MEETINGS WOULD INCLUDE BUT NOT LIMITED TO CITY COUNCIL MEETINGS, COUNTY COMMISSION MEETINGS AND LOCAL NOT FOR PROFIT ORGANIZATIONAL MEETINGS (I.E. KIWANIS, LIONS CLUB INTERNATIONAL, UNIFIED SCHOOL DISTRICT BOARD MEETINGS). TASK 2 - FINANCIAL TRACKING THE CHEROKEE COUNTY HEALTH DEPARTMENT WILL SUBMIT DRAWS FOR ACTUAL COSTS INCURRED TO EPA FOR PAYMENT ON A QUARTERLY BASIS. TASK 3 - REPORTING THE CHEROKEE COUNTY HEALTH DEPARTMENT WILL SUBMIT QUARTERLY REPORTS TO THE EPA PROJECT OFFICER WITHIN 60 DAYS OF QUARTERS END.
