About Proactive Community Service

Proactive Community Service (PCS) is a non-profit organization dedicated to providing health and social services to the community. PCS believes that healthy families lead to healthy outcomes in life, and strives to support individuals and families through various programs and services.

PCS has been awarded a $600,000 Project Grant from the U.S. Department of Health and Human Services Substance Abuse and Mental Health Services Administration (SAMHSA) for its Self-Empowerment through Engagement with Resources (SEER) program. The SEER program aims to provide prevention, testing, and care services to populations at high-risk for substance use disorders and HIV infection in Cook County, Illinois, with a focus on African American transgender, cisgender, and gender nonconforming individuals. PCS will coordinate these services through partnerships with direct service providers including the South Suburban Council on Alcoholism and Substance Abuse, Kirby Rehabilitation, Chicagoland Infectious Disease, and SAJ Family Practice. The program aims to annually serve 500 unduplicated participants through testing, treatment referrals, and linkages to essential services, with the goal of decreasing substance use, risky sexual behaviors, and mental health problems in the target population. Additionally, PCS has received sub-grants from Cook County for the Public Service Proactive Employment Solutions program, and from the Alliance To End Homelessness to actively participate in the Youth Coordinated Entry (CE) system, which involves outreach, diversion, prevention, housing, and other related services for unaccompanied youth experiencing homelessness in Suburban Cook County.
