About this program
The Centers for Medicare and Medicaid Services (CMS) awarded Cooperative Agreements totaling $37.08 billion to all 50 U.S. states under the Rural Health Transformation (RHT) Program, CFDA 93.798, on December 29, 2025, with project completion targeted for October 30, 2030.
The program funds state-led transformation of rural healthcare delivery systems through five aligned strategic goals: improving population health outcomes, expanding sustainable access to care, developing rural healthcare workforces, advancing innovative care models, and deploying health technology. Awarded states are implementing initiatives spanning primary and specialty care access, workforce recruitment and retention, telehealth and digital infrastructure modernization, chronic disease and behavioral health management, maternal and pediatric health services, emergency medical services enhancement, and value-based payment model adoption. Representative awards include Ohio ($808 million) for rural health innovation hubs and EMS transformation; Florida ($1.049 billion) for modernizing rural health systems across 31 counties serving 1.2 million residents; Texas ($1.406 billion) for technology-supported rural wellness and workforce revitalization; and California ($1.168 billion) for regional care collaboratives anchored by critical access hospitals and clinics.
The program operates exclusively as Cooperative Agreements (100% of awards), making states formal partners in implementation alongside CMS. CMS is the sole funding source across all 50 awards. Typical award magnitudes range from approximately $346 million to $1.406 billion over the five-year period, with most states receiving authorization to deploy $200 million annually. This is a one-time appropriation; no renewals or extensions are available. Grant accounting records must be maintained three years after submission of the final Federal Financial Report, or longer if litigation or audit action remains pending. The most recent opportunity (CMS-RHT-26-001) posted September 3, 2025, with application due November 5, 2025, and was restricted to state governments; that solicitation window has now closed as all awards were made December 29, 2025.
