About this program
The Department of Health and Human Services Administration for Community Living funds evidence-based falls prevention programs for older adults and adults with disabilities through CFDA 93.761, with the most recent grant award made September 1, 2025.
The program has maintained consistent activity across 112 awards since 2014, with combined federal dollars obligated at $67 million. Recent award cadence has fluctuated (11 awards in 2021, 7 in 2022, 13 in 2023, 6 in 2024, and 3 in 2025), but the program continues to disburse funding regularly. The portfolio consists predominantly of Cooperative Agreements (85 awards, 95 total) with a smaller share of Project Grants (17 awards). All funding concentration resides within the Administration for Community Living, which obligated $67 million across 112 awards.
Funded activities center on scaling access to evidence-based falls prevention curricula—specifically programs such as A Matter of Balance, BINGOCIZE, Stay Active and Independent for Life (SAIL), and Tai Chi for Arthritis and Falls Prevention—delivered in-person and virtually to reach rural, frontier, homebound, and underserved older adult populations. Recent awards demonstrate focus on statewide expansion, regional coordination, and sustainability infrastructure. The Montana Falls Prevention Program (awarded $2.5 million in September 2025) aims to reach 1,890 participants over three years across three evidence-based programs in rural and frontier regions. The South Dakota–Southwest Minnesota Stand Strong initiative (awarded $3.75 million in September 2025) targets economically and socially challenged older adults and those with disabilities across urban, rural, and frontier communities. The Ohio program (awarded $1.25 million in September 2025) will serve 6,700 participants in A Matter of Balance and Tai Chi programs. Typical award magnitudes range from $500 thousand to $3.75 million for three-year periods.
Two grant opportunities are currently open. Both opportunities accept state governments, tribal entities, 501(c)(3) nonprofits, higher education institutions, and local government applicants. A third opportunity, Expanding Strategies for Community Evidence-Based Falls Prevention Programming (HHS-2025-ACL-AOA-FPSG-0037), closed August 15, 2025.
