About this program

The Centers for Medicare and Medicaid Services (CMS), through its Center for Medicare and Medicaid Innovation, funds state-level cooperative agreements to design and implement integrated care delivery for pregnant and postpartum Medicaid beneficiaries with opioid use disorder (CFDA 93.687). The program is closed to new applicants; CMS currently funds eight renewable one-year cooperative agreements to State Medicaid agencies, and no additional awards are anticipated.

The most recent grant opportunity (CMS-2A2-20-001) was posted in February 2019 with a due date of May 6, 2019, and all awards were made on January 1, 2020. The program has not issued new solicitations since that date. Grant activity is not ongoing; the source indicates that the initial eight state recipients received awards in 2020 and are operating under renewable one-year agreements within a five-year performance period that began then.

Funded activities center on infrastructure and capacity development, care delivery innovation, and achievement of quality metrics tied to maternal and infant health outcomes. Award recipients are State Medicaid agencies that design state-specific interventions—typically multidisciplinary care coordination models, integrated substance use disorder treatment into prenatal and obstetric settings, medication-assisted treatment (MAT) access, enhanced behavioral health services, and systems to minimize separation of mother and infant during hospitalization. Recent awards supported programs in Colorado (integrating SUD treatment into primary and obstetric care sites with regional subgrants up to $506,778 per site), Indiana (value-based purchasing and social determinants of health screening), Texas (hub-and-spoke model centered on Ben Taub Hospital in Houston), West Virginia, Tennessee, Maine, and New Hampshire. The program is 100 percent Cooperative Agreements.

The portfolio shows 11 total federal grant awards obligating $85.4 million in federal dollars and $87 million combined (federal plus non-federal share), all disbursed on January 1, 2020. CMS was the funding agency for all awards. Representative award magnitudes ranged from $298 thousand to $15.8 million, with larger awards to states with extensive implementation scope. Maine received $13.7 million, New Hampshire $13.9 million, Texas $15.8 million, and Indiana $12.8 million. Smaller awards included Missouri at $297 thousand.

This program is operationally closed to new applicants under the current NOFO cycle. Organizations seeking to engage with state-level maternal opioid use disorder interventions should monitor whether CMS issues new solicitations or consider participation as subgrantees or care delivery partners within existing state awards.

Objective and description

Objective

The Centers for Medicare & Medicaid Services (CMS), through its Center for Medicare and Medicaid Innovation (Innovation Center), seeks applications to the Maternal Opioid Misuse (MOM) model. The MOM model is the next step in CMS' multi-pronged strate

Description

The MOM model leverages Innovation Center authorities and state flexibility to address fragmentation in the care of pregnant and postpartum Medicaid beneficiaries with opioid use disorder. To foster integrated and coordinated care for the model population, CMS supports awardees to design and implement state-specific interventions through funding for 1) infrastructure and capacity development, 2) care delivery, and 3) milestones tied to key quality metrics. During the model's five-year performance period, responsibility for funding the care-delivery innovation has transitioned to states, with the ultimate goal of sustaining successful payment and care-delivery strategies through incorporation into their Medicaid programs. CMS currently funds eight renewable one-year cooperative agreements to State Medicaid agencies. No additional awards are anticipated.

Eligibility

Applicant eligibility

State Medicaid agencies are eligible to apply for MOM model funding.

Beneficiary eligibility

State Medicaid Agencies may apply for MOM model assistance.

Uses of assistance

The MOM model provides three distinct funding opportunities for awardees: • Implementation Funding will be available annually during each year of the model to support infrastructure and capacity-building. • Transition Funding will be available only during the Transition Period and only to support intake, assessment, and creation of a treatment plan (payment available once per pregnancy per beneficiary) and coordination, engagement, and referral activities (payment available on a monthly basis for up to 12 months for each beneficiary who receives these services) not otherwise covered by the State Plan. Model funds may not be used to supplant or duplicate existing funding sources. The state must take over these costs in alignment with its Coverage and Payment Strategy by Year 3 of the model. Transition Funding is restricted and not guaranteed. • Milestone Funding will be available during the Full Implementation Period based on an awardee's demonstrated performance on pre-determined quality milestones. Milestone Funding is restricted and not guaranteed. Applicants must describe in detail how they will use Implementation Funding and Transition Funding to implement the model. In addition to the detailed justifications for use of Implementation Funding and Transition Funding, each applicant should provide a plan for how Milestone Funding, if unrestricted, would be used to support the goals of the MOM model.

Length and time phasing of assistance

MOM model funding will be available on an annual basis to awardees for five years. The model will start in FY 2020 and end in FY 2024.
