About this program
The Department of Veterans Affairs Veterans Health Administration funds nursing home care for eligible veterans in State Veterans' Homes under Assistance Listing 64.015, with the most recent grant award issued April 18, 2025.
The program disburses formula grants exclusively to states and Indian tribes that operate designated facilities providing skilled nursing care, intermediate care, or combined nursing facilities for veterans. Eligible veterans include those with service-connected disabilities, former prisoners of war, Purple Heart recipients, Medal of Honor recipients, and veterans unable to defray necessary care costs. The program does not fund domiciliary care. Grant payments flow monthly or quarterly with no length limitation on care duration; recipients must maintain cost records and attendance documentation to support per diem claims.
The Department of Veterans Affairs Veterans Health Administration Under Secretary for Health dominates funding, obligating $59 billion across 192 awards; the broader Veterans Health Administration contributed $245.5 million across 139 awards. The remaining two awards totaling $8.6 million came from the Department of Veterans Affairs. All 333 awards in the portfolio are formula grants. Combined federal obligation across the program is $59.25 billion, with combined total obligation (federal plus state matching) at $60.11 billion. Recent awards show substantial per-state commitments: Formula Grant VA5909AF obligated $117.2 million (April 2025), Formula Grant MN656 obligated $57.6 million (December 2024), and Formula Grant AL6199AG obligated $62.4 million (October 2024). Award cadence has slowed recently—one award in 2025, three in 2024, and one in 2023—following higher activity in prior years.
