About this opportunity
Livingston County, New York is soliciting proposals for supplemental rehabilitation staffing and consulting services for the Livingston County Center for Nursing and Rehabilitation and Garden of Life Adult Day Health Care Program. The Request for Proposals (RFP) seeks a vendor to provide physical, occupational, and speech therapy services to supplement existing in-house rehabilitative services for short-term and long-term patients. The selected vendor will be required to provide software for electronic documentation, generate reports, and ensure compliance with Medicare and Medicaid guidelines. The procurement is for an initial one-year contract with options to renew for four additional one-year periods. Proposals will be received until 11:00 a.m. on November 13, 2025, at the Livingston County Government Center. The evaluation criteria will assess proposals based on scope of work (35 points), cost (30 points), staff qualifications (30 points), and quality assurance program (5 points). Interested vendors must register with Ashley Scutt by emailing ashleyscutt@livingstoncountyny.gov or calling (585) 243-7040 to ensure receipt of any potential addendums.
The facility is a 266-bed public, non-profit residential healthcare facility with a current average occupancy rate of 94.78% and an existing therapy staff that includes 2 Physical Therapists, 3 Physical Therapy Assistants, 1 Occupational Therapist, 3 Certified Occupational Therapy Assistants, 1 Speech-Language Pathologist, and 1 Program Director. While no specific set-aside designations are mentioned, Livingston County is an equal opportunity/affirmative action employer. The current payor mix consists of approximately 70% Medicaid and 8.1% Medicare Part A rehabilitation, with a 2025 average Rehab PPS PDPM of approximately $560.00 per day. The facility includes specialized units such as a 26-bed Transitional Care Unit, a 12-bed Bariatric Care unit, a 48-bed Memory Care unit, and a 20-slot Adult Day Health Care Program. The county reserves the right to reject any and all proposals, and while no specific budget range is provided, the comprehensive nature of the services suggests a significant potential contract value for the selected vendor.
