About this opportunity
The Suffolk County Office of Central Procurement, on behalf of the Suffolk County Office for the Aging, is soliciting proposals for Social Adult Day Care services under the Older Americans Act Title IIIE Family Caregiver Support Program through Request for Proposals 2025-36. The procuring agency seeks qualified entities to provide comprehensive social model adult day care services for functionally impaired individuals aged 60 and older and individuals with Alzheimer's disease or related disorders regardless of age in a congregate protective setting operating Monday through Friday from 8:00 a.m. to 2:00 p.m. Required services include four mandatory components—socialization, supervision and monitoring, personal care, and nutrition—with optional services including transportation, caregiver assistance, and case coordination. Proposers must demonstrate a minimum of two years of experience operating a Social Adult Day Care program and maintain compliance with New York State Office for the Aging regulations (9 NYCRR 6654.20), including individualized service care plans reviewed at least annually and monthly reporting on performance measures. The evaluation criteria allocate 40 points to general qualifications and proposer history, 40 points to proposed technical services, and 20 points to cost proposals, with cost proposals presented on a daily rate basis both with and without transportation. A non-mandatory proposer's conference is scheduled for December 8, 2025, at 10:00 a.m. via video conferencing, with technical questions due by December 9, 2025, at 3:30 p.m., and proposals must be submitted online through the Bonfire portal by January 5, 2026, at 3:30 p.m. The contract term commences January 1, 2026, and runs through December 31, 2026, with four optional one-year renewal periods available through December 31, 2030.
Respondents must be New York not-for-profit corporations or municipal corporations to be eligible for contract award. The total available funding is $60,000 annually, with a maximum of $15,000 available to any single successful proposer per year, structured with 75 percent federal support through the Administration for Community Living and 25 percent county funds. Contractors are required to provide a minimum of 10 percent agency matching funds from non-county resources, with the match amount deducted from total reimbursement. Proposers must submit comprehensive compliance documentation including certified financial statements from an independent CPA, client references from the past three years, and various Suffolk County and New York State compliance forms, and all proposals must remain effective for at least 180 days from the proposal due date. Additionally, contractors must comply with federal subrecipient obligations under 2 CFR Part 200, Suffolk County living wage requirements (minimum $17.21 per hour with health benefits or $17.36 without benefits for covered employees), lawful hiring practices, union organizing law compliance for contracts exceeding $50,000, and comprehensive insurance and audit requirements as specified in the model contract documents.
