About this opportunity
Leon County and the Florida Department of Elder Affairs are pursuing a single-source procurement for the ElderCARE! program through Navigating Aging Needs, LLC (NAN), targeting support for the Florida Alzheimer's Center of Excellence (FACE). The procurement will provide subscription-based services for caregivers of persons living with dementia, specifically focusing on individuals on the Alzheimer's Disease Initiative waitlist. Key deliverables include three-month subscriptions with potential renewals, personalized care plan development, access to support systems, and establishment of a comprehensive caregiver registry for academic research and outcomes tracking. The program aims to offer clinical and expert-level support beyond the current FACE program capabilities, addressing complex family challenges associated with dementia care. The contract is scheduled for a one-year term with a posted date of April 24, 2025, and a proposal due date of May 15, 2025. Evaluation criteria appear to center on the vendor's unique expertise, particularly the specialized backgrounds of NAN's co-founders Dr. Rosemary Laird and Patricia Allman in senior care and public-private partnerships.
The procurement is designated as a single-source contract with no explicit set-aside provisions for disadvantaged enterprises. The total contract value is $1,000,000, with NAN being the sole anticipated vendor due to their proprietary care navigation platform that tracks over 80 factors related to patient and caregiver well-being. The program will involve partnerships with an insurance provider and a local healthcare provider to develop the Care Navigation program. NAN's selection is justified by their demonstrated efficacy through a pilot project in Broward County and recognition including findings presented at the Alzheimer's Association International Conference. The procurement targets a critical demographic need, with approximately one-quarter of Florida's current residents being baby boomers approaching retirement age. Integration requirements include connecting with existing FACE program infrastructure and developing a unique caregiver registry that will track patient and caregiver data longitudinally over one year to analyze intervention impacts.
Opportunity text
The ElderCARE! program will offer high-level support to the Florida Alzheimer's Center of Excellence (FACE), a program of the Department of Elder Affairs. It will provide routine, supportive, and preventive care through NAN subscriptions to Frail Caregivers of the Elderly (FCE) on the Alzheimer's Disease Initiative (ADI) waitlist and/or being supported by the FACE program. The ElderCare! program will serve as a clinical and expert level of support to Persons Living With Dementia (PLWD) and/or their family caregiver when the challenges experienced by the family are more complex than the FACE program is able to address. The service includes subscription-based support for Persons Living With Dementia and their caregivers through assessment and personalized development of care plans and access to supplemental personal and virtual support systems. Subscriptions are for three months and can be renewed should the PLWD and their caregiver require additional assistance. When the family has reached stability within their careplan, they will be referred back to the FACE program for ongoing, non-clinical support. Additionally, a caregiver registry will allow for further academic research and outcomes tracking. The NAN Caregiver Registry is unique in that it will track both FCE and Alzheimer's disease patient data over one year - before, during, and after interventions are implemented. This registry creates a rich data source for analyzing the impact of specific interventions on Alzheimer patients and their FCEs. Additionally, it will provide valuable long-term insight into the most effective interventions to reduce the burden of care and prevent FCE burnout.
