About this opportunity
The State of Oregon, Oregon Health Authority, is requesting grant proposals for the Oregon Health Plan Health System Navigation Grant Program under RFGP 6291, with proposals due November 3, 2026 at 3:00 p.m. Pacific Time. OHA intends to award 40–45 grants at a maximum of $150,000 per grant for a 24-month performance period from July 1, 2027 through June 30, 2029.
Eligible applicants are non-profit 501(c)(3) organizations, hospitals, safety net clinics, public health departments, and schools. Organizations receiving Medicaid Administrative Claiming or Medicaid outreach, enrollment, and education funds are ineligible unless they demonstrate that funding will not support the same activities and maintain separate financial tracking. Ineligible entities include insurers, insurance subsidiaries, insurance industry associations, and any organization receiving compensation from health insurers.
The program focuses on partner-based outreach and direct enrollment assistance for individuals and families eligible for Oregon Health Plan coverage, with system navigation services available to all consumers regardless of eligibility status. Grant activities must allocate 90% of funding to health system navigation and 10% to outreach. Required activities include OHP application assistance, staff training on health system navigation, health system navigation education delivery, medical appointment scheduling, and connection of members with supportive services. All grantees must comply with community-responsive and linguistic competency standards, organization standards, and application assister and community health worker standards, including language access services, criminal history checks, and privacy/consent procedures.
Budget line items must separate personnel salaries and benefits, travel at current Oregon state rates, training, outreach and enrollment services, supplies (capped at 20% of total budget), and indirect/administrative expenses (capped at 15% of budget subtotal). No single month's expenses may exceed 9% of the total award amount. Questions are due October 7, 2026 at 1:00 p.m., with answers provided by approximately October 14. The Notice of Award is estimated for January 1, 2027. Contact Jewelee Bell at 971-208-4144 or jewelee.m.bell@odhsoha.oregon.gov.
