About this opportunity

The Maryland Department of Health, Center for Chronic Disease Prevention and Control, is requesting proposals to enhance enrollment in Diabetes Self-Management Education and Support (DSMES) programs across Maryland under RFP PHPA 4014, with proposals due August 27, 2026 at 2:00 PM EST. The Department intends to make up to two awards. A pre-proposal conference is scheduled for August 18, 2026 at 2:00 PM EST, and written questions are due August 20, 2026 at 2:00 PM EST.

Offerors must be accredited DSMES programs through the Association of Diabetes Care and Education Specialists (ADCES) or recognized by the American Diabetes Association, with proof of accreditation or recognition required at proposal submission — the mandatory capability filter. Minority Business Enterprises and Small Business Reserve entities are encouraged to respond.

The scope requires implementing standardized referral and enrollment processes, provider engagement strategies, and workflow improvements to expand program reach and increase enrollment among individuals with diabetes. Deliverables include a comprehensive work plan, site assessment report, provider training documentation, baseline and quarterly reports, and a final report with outcome data and sustainability recommendations.

The contract is structured as firm fixed-price, running from October 1, 2026 through June 29, 2027. Financial proposals must detail all direct costs including salaries, fringe benefits, consultant costs, training, supplies, travel, contractual services, and software; indirect costs may not exceed 10% of total direct costs. Program support incentives are limited to $25 per participant; assistance with healthy foods or transportation cannot exceed $80 per person. Prohibited costs include diabetes awareness campaigns duplicating CDC resources, programs lacking ADCES accreditation or ADA recognition, cash or gift card incentives, costs reimbursed by other payers, food, lobbying, medical equipment, and salaried healthcare professional services for direct DSMES delivery.

Award will be made to the responsible offeror whose proposal is most advantageous to the State, considering technical and financial factors equally. This procurement follows small procurement regulations (COMAR 21.05.07). Proposals must be submitted electronically through eMaryland Marketplace Advantage (eMMA).
