The Maryland Department of Health, Center for Cancer Prevention and Control, is issuing a Request for Applications to fund Federally Qualified Health Centers, primary care clinics, and health systems implementing evidence-based interventions and quality improvement methods to increase colorectal cancer screening rates for populations aged 45 to 75 years at clinics with current screening rates below 60%, with applications due July 26, 2026 at 2:00 PM EST.
Applicants must be nonprofit 501(c)(3) entities or government agencies in good standing with the Maryland State Department of Assessments and Taxation. Applicants must provide proof of nonprofit status, a signed W-9, and maintain commercial general liability insurance of $1 million. Up to two awards will be made.

Mandatory activities include patient reminders, provider reminders, and patient navigation. Optional activities include reducing structural barriers and provider assessment with feedback. Applicants must modify electronic health records to track screening rates and related metrics. Applicants must submit quarterly invoices, monthly expenditure trackers, annual clinic data, equity assessments, and sustainability plans.
Funding is calculated at $50,000 minimum per year plus $8 per eligible patient aged 45–75 years, capped at $123,450 annually. No single award exceeds $123,450. Eligible budget categories include salaries, fringe benefits, consultants, EHR contractual services, supplies, transportation, postage, printing, and indirect costs not exceeding 10% of direct costs. Prohibited expenses include food, lobbying, medical equipment, direct clinical services, and promotional materials.
The contract term spans Year 1 plus three option years through June 30, 2030, contingent on performance and fund availability. Funding is provided by the Centers for Disease Control and Prevention through MDH. A pre-application conference is scheduled for July 9, 2026. Questions are due by July 17, 2026.
