All submissions must include the completed HHS Solicitation Affirmations and Pricing Sheet (Exhibit C); failure to provide required documentation or supporting literature for equivalent products may result in disqualification. Both HHSC and the Department of State Health Services (DSHS) are entitled to utilize any resulting contract. Bidders may submit responses through either the HHS Online Bid Room (accessible through the IAMOnline system with advance registration required) or through alternative submission methods specified in the solicitation package. The contract may be subject to price adjustments tied to the Consumer Price Index as published by the Bureau of Labor Statistics, though adjustments remain at HHSC's sole discretion.
