This is an emergency procurement with immediate operational requirements; the contract term runs November 16, 2026 through November 15, 2027 with no renewal options.
Respondents must demonstrate five years of experience in skilled nursing care, good standing with regulatory agencies, active SAM.gov registration, and financial solvency. Services encompass comprehensive facility management including medical and nursing care, rehabilitative therapy, dental services, social services, pharmaceuticals, psychological services, nutrition and dining programs, janitorial and laundry services, maintenance and repairs, personnel and staffing management, and quality assurance programs. The facility includes 30 beds designated for Alzheimer's care. Operations must comply with VA regulations under Title 38 C.F.R. Part 51 and Texas Health and Human Services Commission regulations. Respondents must use PointClickCare for information systems and maintain resident trust fund management and financial controls.
The cost proposal structure comprises fixed monthly fees and variable fees based on resident census across multiple payor sources: VA per diem (currently $148.71 daily subsidy), private pay, Medicare A, Medicare B, Medicare B co-insurance, and Medicaid. Established daily rates by GLO/VLB range from $151.60 to $218.60 per day depending on room type and memory care designation. The current operator is STG International, Inc.
Evaluation prioritizes proposed management and operations methodology (30 percent), personnel and staffing methodology with key staff qualifications and a staffing plan to maintain 95 percent occupancy (25 percent), reasonableness of cost (20 percent), and comparable veterans-centered experience (15 percent), with 10 percent for overall responsiveness. Respondents must provide three references for similar contracts completed within three years, preferably with state or local government entities, and complete a VetHUB subcontracting plan.
