About this opportunity
Hawaii Department of Health's Emergency Medical Services & Injury Prevention Systems Branch is soliciting bids for evidence-based falls prevention exercise programming for kupuna (older adults) in the Waianae District, with proposals due August 17, 2026 and performance beginning August 20.
The scope requires weekly STEADI-aligned exercise classes over a 6-week period (August 20 through September 30, 2026), with the State reserving the right to extend for one additional 6-week period at equal or lower cost. Sessions must accommodate a minimum of 10–15 participants and include sit-to-stand practice, balance training, gait training, and core strengthening with modifications for participants using mobility aids. Each class incorporates 5–10 minutes of rotating fall-prevention education (home hazards, safe footwear, assistive device use, medication management) in large-print, high-contrast formats. The contractor must embed individual fall-risk screening using STEADI-recommended tools — chair rise testing, balance stance assessment, and Timed Up and Go testing — and submit monthly reporting of attendance, screening results, and participant progress to the Department of Health.
Bid prices are all-inclusive of applicable taxes and expenses, submitted through the HIePRO system using the contractor's exact legal name as registered with the Hawaii Department of Commerce and Consumer Affairs. Award goes to the responsive, responsible contractor submitting the lowest offer. Prior to contract execution, the contractor must submit a valid tax clearance certificate from the Hawaii State Department of Taxation and the Internal Revenue Service (green certified copy stamp valid for six months), maintain membership in good standing with Hawaii Compliance Express, and provide certificates of insurance with Commercial General Liability coverage of $1 million combined single limit per occurrence and $2 million general aggregate, plus Automobile Liability coverage of $1 million combined single limit per occurrence. Either party may terminate with two weeks' written notice. Work does not commence until an authorized, signed Purchase Order is issued by the State of Hawaii.
