About this opportunity

The Maryland Department of Health, Office of Minority Health and Health Disparities, is requesting applications for community-based interventions addressing social determinants of health and obesity in Maryland under RFA BPM055359, with applications due March 11, 2026 at 5:00 PM via eMaryland Marketplace Advantage. Applicants must be Maryland-based nonprofit organizations with 501(c)(3) status, local or state government agencies, public colleges, or state universities, and must maintain an operational office within the proposed service jurisdiction.

The scope requires enrollment, client needs assessment regarding social determinants of health, referrals to SDOH services, and follow-up assessments. Grantees may provide direct services in housing instability, food and nutrition, transportation, primary care access, job opportunities, education, violence reduction, or obesity. Two-part proposals consist of a technical narrative (maximum 10 pages) and a separate budget proposal using specified MDH forms. Applicants must submit proof of tax-exempt status, a Maryland Certificate of Good Standing, registration on eMMA, and a signed W-9 form.

The Department anticipates making 15 to 25 awards ranging from $50,000 to $200,000 each for a one-year period from July 1, 2026 to June 30, 2027. Grants are reimbursement-based with no advance payments; grantees must fund the first three months of operations from their own resources. Quarterly invoices are due 15 days after quarter-end, with approximately 30 days required for processing.

Evaluation criteria weight program design at 70 percent and operational details at 30 percent, with budget reasonableness evaluated separately. MHHD will conduct at least one site visit during the grant period, and grantees must submit monthly reports by the 10th of the following month and an end-of-year report by August 31, 2027. The Department reserves the right to cancel the RFA, reject any or all applications, and terminate contracts with 90 days' notice if grantees fail to meet obligations. A pre-application webinar is scheduled for February 26, 2026 from 12:00 PM to 2:00 PM.
