The procurement seeks qualified organizations to provide High-Fidelity Assertive Community Treatment (ACT) and Intensive Case Management (ICM) services to approximately 400 participants with serious mental illness and/or co-occurring substance use disorders across one or more of five geographic service regions within San Bernardino County: East Valley, West Valley, High Desert, Mountain Region, and Morongo Basin. Required services include evidence-based, person-centered, recovery-oriented comprehensive integrated treatment for mental health and substance use disorders; Individual Placement and Support employment services; Housing First model interventions; psychiatric support with 24/7 crisis intervention capabilities; nursing services; peer specialist support; vocational specialists; and wellness center linkages. Proposers must maintain multidisciplinary teams meeting TMACT fidelity standards with staffing ratios of 10:1 for ACT and 15-20:1 for ICM, provide minimum participant contact of two per week for ICM and multiple contacts weekly for ACT, maintain 24/7/365 on-call coverage with office availability seven days per week, and obtain or maintain Medi-Cal certification. Services will be reimbursed on a fee-for-service cost reimbursement basis utilizing Medi-Cal, Mental Health Services Act (MHSA), Behavioral Health Services Act (BHSA), and 1991 Realignment funding sources. Questions must be submitted by March 30, 2026, at 12:00 noon PST, with a mandatory proposer conference scheduled for March 24, 2026, at 2:00 PM PST via Microsoft Teams. Proposals are due by 4:00 PM PST on April 8, 2026, via ePro electronic submission or hard copy delivery to DBH Administration at 303 E Vanderbilt Way, San Bernardino, CA 92415. Board of Supervisors contract award presentation is scheduled for June 23, 2026, with services commencing July 1, 2026. Evaluation criteria consist of 65 percent program components and 35 percent financial components, including program design, organizational capacity, financial statements, cost per participant, and debt ratios.
Eligible respondents must be nonprofit or for-profit organizations with current Medi-Cal certification or the ability to obtain it within the contract period, certification as a Long-Term or Transitional Adult Social Rehabilitation Residential Facility, and no record of unsatisfactory government performance within the preceding 24 months. Proposers must submit three years of audited financial statements or unaudited statements if audited statements are unavailable, maintain single audit compliance if expending $1,000,000 or more in federal funds annually, and comply with indirect administrative cost caps of 15 percent of modified direct costs. Required insurance coverage includes general liability of $1,000,000 per occurrence and $2,000,000 aggregate, professional liability of $1,000,000 per occurrence and $2,000,000 aggregate, and abuse/molestation coverage at the same limits, with certificates of insurance required within fourteen calendar days of contract award. The RFP does not identify specific incumbent vendors or set-aside designations for disadvantaged enterprises. The total contract value is $8,116,130 annually or $40,580,650 for the five-year aggregate term, with budget schedules provided for fiscal years 2026-27 through 2030-31 establishing Medi-Cal funding at 50 percent and MHSA/BHSA funding at 50 percent, though the County reserves discretionary authority to award contracts to one or more proposers and allocate funding as determined appropriate subject to Board of Supervisors approval. All service areas must be collectively covered if multiple providers are awarded, and proposers may elect to bid on one or multiple regions without disadvantage to those proposing less than all five areas.
