About this opportunity
The State of Maine Division of Purchases is requesting quotations for epidemiology business cards for the Maine Center for Disease Control & Prevention, with quotations due January 13, 2014. The solicitation is structured as an RFQ submitted electronically through the State's Vendor Self Service platform, closing at 2:00 p.m. local time.
The scope is the design and production of business cards incorporating QR code functionality and Maine CDC branding, with contact information for disease reporting (disease.reporting@maine.gov and 207-287-6865). Deliverable specifications are provided in the QR Code Template design document.
A recycled-content price preference applies: bids offering paper products with a minimum of 30% post-consumer recycled material that fall within 10% of the lowest compliant bid may receive up to a 10% price preference adjustment. Vendors must explicitly identify recycled-content offerings and disclose the specific recycled-content percentage in their response. Separately, the State offers a premium of up to 4% above the lowest bid for paper products with 100% post-consumer recycled or certified virgin fiber content (from ATFS, FSC, MLCP, PEFC-affiliated entities, or SFI); if no bids meet the 100% threshold, preference is given to the highest percentage of recycled or certified virgin fiber content within the 4% cost cap. Bidders must provide chain-of-custody certification or equivalent proof for each paper product offered and identify certification labels.
The contract term does not exceed three years, with the State Purchasing Director retaining the right to extend beyond the stated expiration date with contractor consent. Bids are subject to a maximum 3% overrun allowance. Vendor past performance may be evaluated as a factor in award. Evaluation criteria weight price at a minimum of 25%, with quality, conformity to specifications, delivery time, and ultimate cost also considered. Pricing is submitted on a unit basis, with unit price governing in case of discrepancies. FOB Destination delivery with freight prepaid by the bidder applies unless otherwise specified. Payment terms are net 30 days from receipt of a correct invoice. No estimated contract value is stated.
