About this opportunity
San Mateo County Health, through its Public Health, Policy & Planning (PHPP) division, is seeking proposals for Community Health Improvement Plan (CHIP) Collaborative Planning Services. The procurement aims to identify a qualified organization to organize, facilitate, and lead the development of a county-wide collaborative focused on advancing health priorities in three key areas: Access to Health Care Services, Mental Health, and Social Determinants of Health. Proposing organizations must have a minimum of 5 years of service history in San Mateo County and 3 years of experience leading multi-organizational collaboratives, with the capacity to provide a full-time Program Coordinator. The selected vendor will be responsible for establishing and guiding the CHIP Collaborative, working closely with the CHIP Steering Committee and Work Groups to ensure diverse community representation and implementation of health improvement strategies. Proposals are due by 5:00 pm on August 22, 2025, with an anticipated award date of November 3, 2025. The evaluation process will assess proposers based on organizational history, cultural competency, service methodology, staff capacity, and budget, with a total of 100 possible points. Additional preferential points will be awarded to local businesses and non-profits, with special consideration for small and micro businesses.
The contract opportunity allows for joint proposals, requiring the primary applicant to have a documented partnership with any secondary applicant through a memorandum of agreement or similar mechanism. Proposers must be registered in the System for Award Management (SAM) and will be expected to allocate funding for Community-Based Organization (CBO) leaders at $45,000 per year and community member stipends at $5,000 per year. The total contract value will not exceed $400,000 for two years of services, with indirect expenses limited to 12% of the total budget. Organizations must comply with equal employment opportunity regulations, non-discrimination policies, and local ordinances such as the Living Wage Ordinance. The county emphasizes authentic community collaboration and requires the contractor to integrate with existing or newly established community collaboratives. Successful proposers will be expected to maintain appropriate insurance coverage, including workers' compensation, general liability, and professional liability, with the county reserving the right to terminate the contract based on funding availability.
