About this opportunity

The South Dakota Department of Health, Office of Chronic Disease Prevention and Health Promotion, is issuing a Request for Proposal (RFP #26-09RHT-004) to procure caregiver support programming services statewide under the Rural Health Transformation Program. The state seeks one or more organizations to provide comprehensive services, education, and resources to caregivers, providers, and the general public, with specific objectives including establishing and expanding peer support groups, coordinating available resources, enhancing provider training, and increasing respite care availability. Qualified offerors must submit a Letter of Intent by March 24, 2026, at 5:00 PM CST to be eligible for consideration; submit technical and cost proposals by April 22, 2026, at 5:00 PM CST via email to Kiley.Hump@state.sd.us; provide at least three references documenting similar prior contracts or services; and demonstrate capability to partner with SD DOH chronic disease programs. The RFP schedule requires offeror questions to be submitted by March 27, 2026, with state responses provided by April 1, 2026. Evaluation criteria prioritize technical qualifications and approach, with cost becoming increasingly important as technical scores converge. The contract period runs for 17 months from June 1, 2026, through October 30, 2027, with three optional one-year renewal periods, with anticipated award and contract negotiation occurring by June 1, 2026.

The RFP does not specify set-aside designations or restrictions limiting organizational types eligible to respond. Offerors must disclose any other Rural Health Transformation funding applied for with South Dakota or other states and must provide detailed cost proposals identifying all service-related expenses, including personnel costs with salary and fringe benefits, supplies, travel at state per diem rates (subject to advance approval), indirect costs capped at 6.3% of the total contract amount or the organization's federally approved rate, and other allowable costs. The state requires contractors to maintain $1,000,000 in commercial general liability insurance, professional liability insurance, business automobile liability insurance, and workers' compensation insurance. Awarded vendors must submit monthly reports on contract deliverables, participate in monthly programmatic meetings, attend training and webinars as directed, and submit itemized monthly expenditure claims within 30 days of month-end. No specific budget range is disclosed in the available RFP materials, and no incumbent vendors or existing contractors are identified.
