Mandatory submissions include proof of required certifications, completed HHS Solicitation Affirmations (Exhibit A), Pricing Sheet (Exhibit C), Bidder Reference and Contract Qualifications Form (Exhibit D), and documentation of financial capability and capacity to perform. Bidders must provide a minimum of three verifiable references for contracts of similar size and scope completed within the last five years, current staffing information, industry certifications and qualifications for performing staff, a one-page narrative of experience and qualifications, and at least two 24-hour emergency contacts. Optional submissions include completed Security and Privacy Inquiry form (Exhibit B-1), Form 4109 Texas Identification Number application if applicable, and addendum acknowledgements if issued.

All respondents must execute HHS standardized contract affirmations addressing suspension and debarment, human trafficking, franchise tax compliance, E-Verify verification, cybersecurity training, and compliance with Texas-specific procurement restrictions. The Data Use Agreement requires breach reporting within 24 hours for confidential information and compliance with NIST security standards. Bids are binding for 240 days from submission. Submissions are accepted in-person at HHS Procurement and Contracting Services, 1100 W. 49th Street, Austin, or through the optional HHS Online Bid Room; use of the online system is voluntary and technical difficulties do not constitute grounds for protest.
