About this opportunity
The Indian Health Service (IHS), a component of the Department of Health and Human Services, is seeking cooperative agreement applications from federally recognized Tribal governments and Tribal organizations for the Tribal Self-Governance Planning Cooperative Agreement Program. The primary objective is to provide resources for Tribes interested in entering or expanding their participation in the Tribal Self-Governance Program (TSGP), supporting comprehensive planning for assuming Programs, Services, Functions, and Activities (PSFAs) from the IHS. Applicants must demonstrate financial stability and the ability to manage finances for three consecutive fiscal years, with the planning phase requiring legal and budgetary research, internal Tribal government planning, and organizational preparation for healthcare program administration. The program is authorized under the Snyder Act, Transfer Act, and Title V of the Indian Self-Determination and Education Assistance Act. The application deadline is August 1, 2025, at 11:59 p.m. Eastern Time, with successful applicants expected to conduct thorough planning to improve negotiation efficiency and ensure readiness for PSFA transfers.
The funding opportunity will award up to three cooperative agreements, with each award valued at $180,000 for a single one-year budget period, totaling $540,000 in expected program funding. Performance locations will be within the respective Tribal communities and organizations, focusing on developing strategies for more culturally appropriate and efficient healthcare delivery. The cooperative agreements are listed under Assistance Listing 93.444 and will support Tribes in conducting comprehensive assessments of current health services, evaluating potential program transfers, and preparing organizational infrastructure to assume greater control over healthcare services.
Synopsis
The purpose of this Planning Cooperative Agreement is to provide resources to Tribes interested in entering the Tribal Self-Governance Program (TSGP) and to existing Self-Governance Tribes interested in assuming new or expanded Programs, Services, Functions, and Activities (PSFA). Title V of the Indian Self-Determination and Education Assistance Act requires a Tribe or Tribal organization (T/TO) to complete a planning phase to the satisfaction of the Tribe. The planning phase must include legal and budgetary research and internal Tribal government planning and organizational preparation relating to the administration of health care programs. See 25 U.S.C. 5383(d). The planning phase is critical to negotiations and helps Tribes make informed decisions about which PSFAs to assume and what organizational changes or modifications are necessary to successfully support those PSFAs. A thorough planning phase improves timeliness and efficient negotiations and ensures that the Tribe is fully prepared to assume the transfer of IHS PSFAs to the Tribal health program.
