About this opportunity

The Department of Health and Human Services Office of the Assistant Secretary for Preparedness and Response (ASPR) is soliciting cooperative agreement applications to fund trauma system readiness and coordination models, with applications due September 18, 2026. Eligible applicants are limited to federally recognized tribal governments, state governments, consortia of states, consortia of Level I, II, or III trauma centers designated by applicable state or local agencies, and consortia or partnerships of nonprofit Indian Health Service, Indian Tribal, and urban Indian trauma centers; one eligible member must submit the application on behalf of the group. State governments and other state entities must provide a 1:3 cost-share match ($1 non-federal for every $3 federal), while Indian Tribes, Tribal organizations, trauma center consortia, and Indian Health Service partnerships are exempt from match requirements.

ASPR will award up to four recipients between $300,000 and $500,000 each, totaling $2,000,000 across fiscal years 2026 through 2028, with at least $300,000 guaranteed in the first year. Applicants must conduct a minimum of two required pilot project activities from six options: strengthen coordination with trauma partners; create mechanisms for patient transfer improvement; improve resource tracking; implement regional data management systems; establish evidence-based clinical and operational protocols; and facilitate clinical research capacity. Merit review weights background and approach at 50 points, evaluation and performance measurement at 20 points, organizational capacity at 20 points, and budget justification at 10 points, with up to 5 additional points available for applicants serving medically underserved populations. Award notifications are expected September 30, 2026, with project start on September 30, 2026. The program operates under CFDA 93.817.

Synopsis

A trauma system is, "…an organized, inclusive approach to facilitating and coordinating a multidisciplinary system response to severely injured patients."[14] Coordination across prehospital entities (e.g., EMS) and hospitals signal an effective trauma system, where patients can get the right level of care they need. Access to trauma care in the United States is inconsistent. For example, 1 in 3 Americans live in a region with incomplete trauma care,[15] and 30 million Americans live over 60 minutes from a trauma care center.[16]

The Trauma Care Readiness and Coordination Program supports ASPR's goal of strengthening state and local preparedness by funding and supporting efforts to improve coordination across EMS, pre-hospital providers, and hospitals.[17] Through this program, ASPR will help recipients test scalable models that improve trauma system readiness and patient access to the right level of care, ultimately improving health outcomes and saving lives.
