About this opportunity

The Health Resources and Services Administration (HRSA) is soliciting applications for the Telehealth Nutrition Services Network Grant Program to support telehealth networks that integrate nutrition services into primary and specialty care in rural and medically underserved areas, with an application deadline of July 8, 2026 at 11:59 p.m. Eastern Time.

Eligible applicants include domestic public and private organizations, 501(c)(3) and non-501(c)(3) nonprofits, faith-based organizations, tribal governments and tribal organizations, state and local governments, special district governments, public and private higher education institutions, independent school districts, small businesses, public or Indian housing authorities, and for-profit organizations. Applicants must establish a telehealth network with at least one distant site and at least two originating sites located in rural and medically underserved areas, with at least one originating site being a community-based health care provider. All originating sites must be located in rural and medically underserved areas. No cost share is required.

HRSA will award approximately 18 grants totaling $5,400,000, with individual awards up to $300,000 per year for five consecutive 12-month budget periods from September 1, 2026 through August 31, 2031. Award notification is expected August 3, 2026.

Applicants must allocate at least 50% of funding for rural activities and deliver nutrition services (including medical nutrition therapy, individual counseling, group classes, or screenings) targeting at least two chronic disease focus areas: diabetes, cardiovascular disease, obesity, cancer, asthma, mental health, or others. Mandatory staffing includes at least 1.0 FTE registered dietitian, 0.5 FTE community health worker, and 0.25 FTE project director. Applicants must demonstrate at least six months of prior telehealth experience with at least 15 patients monthly and prior nutrition service delivery. Programs must align with Make America Health Again (MAHA) priorities.

Merit review evaluates applications on six criteria: Need (10 points), Response/Methodology and Work Plan (35 points), Performance Reporting and Evaluation (15 points), Impact and Sustainability (15 points), Resources and Capabilities (20 points), and Support Requested/Budget (5 points). Funding preferences apply to rural or community-based organizations, programs addressing mental health or long-term care, projects coordinating with other federal programs, networks including public libraries or educational institutions, and projects promoting local and regional connectivity.

The program operates under Assistance Listing 93.468 (Telehealth Direct Clinical Services).
