About this opportunity
The Department of Health and Human Services Health Resources and Services Administration is soliciting applications for five-year grants to strengthen rural health care delivery systems through State Offices of Rural Health, with an application deadline of July 10, 2026. Only one applicant per state is eligible, designated by the governor, and the program is open to state governments exclusively.
The expected total available funding is $12,110,500, with approximately 50 grants anticipated at up to $243,410 per award. Awards cover five 12-month budget periods from July 1, 2026 through June 30, 2031. Recipients must contribute 75% of the federal award amount as non-federal matching funds through cash or in-kind contributions. Indirect costs are limited to the lower of 15% of total direct costs or the approved negotiated indirect cost rate.
Required activities include maintaining a clearinghouse for collecting and sharing information on rural health care issues and research findings, coordinating rural health activities to avoid duplication, and identifying federal and state programs to help public and nonprofit entities participate in them. Recipients may conduct workforce recruitment and retention activities and provide contracts to public and nonprofit organizations.
Merit review uses six weighted evaluation criteria: Need (20 points), Response (35 points), Performance Reporting and Evaluation (10 points), Impact (10 points), Resources and Capabilities (15 points), and Support Requested (10 points). Applications are limited to 40 pages for narratives and attachments and must include project narrative, budget narrative, work plan, staffing plan, job descriptions, biographical sketches, partner agreements, multi-year budgets, organizational chart, and supporting documents.
Recipients must align funded activities with HRSA strategic priorities including Making America Healthy Again (MAHA), child protections, evidence-based health care, and public safety objectives, and comply with 2 CFR Part 200 and the HHS Grants Policy Statement. The CFDA number is 93.913.
Synopsis
The State Office of Rural Health (SORH) Program assists states in strengthening rural health care delivery systems by maintaining a focal point for rural health. This program funds an institutional framework in each state that links small rural communities with state and federal resources to help develop long-term solutions to improve access to, and the quality of care for the nearly 60 million people living in rural communities.
