The primary objective is to empower Medicare beneficiaries, their families, and caregivers to prevent, detect, and report healthcare fraud, errors, and abuse through targeted outreach, counseling, and education. Key contract requirements include developing a user-friendly website, supporting team member recruitment and training, creating fraud prevention awareness materials, and providing comprehensive technical assistance to grantees. Evaluation criteria will focus on project relevance, approach, budget, project impact, and organizational capacity. Critical dates include a letter of intent deadline of July 10, 2025, and a full application due date of July 30, 2025, with an anticipated project start date of September 1, 2025.

The funding opportunity offers a cooperative agreement with $1,500,000 allocated for the first year and $1,000,000 for each subsequent year, with an additional $500,000 specifically designated for testing new technologies in Year 1. The project period is structured as a three-year cooperative agreement, involving weekly meetings with the ACL Project Officer and requiring flexibility in supporting the nationwide network of SMP projects. The grant falls under CFDA Number 93.048 and is open to a wide range of applicant types, including state and local governments, tribal organizations, non-profit entities, and institutions of higher education, with the primary performance location being domestic and supporting projects across all 50 states, the District of Columbia, Puerto Rico, U.S. Virgin Islands, and Guam.
