About this opportunity

The Indian Health Service is forecasting a cooperative agreement to fund Public Health Nursing case management and care navigation services for American Indian and Alaska Native men, with no hard application deadline specified in this forecast notice.

Eligible applicants are federally recognized Indian tribes, tribal organizations, and urban Indian organizations as defined under 25 U.S.C. sections 1603 and 1603(26), and the Indian Self-Determination and Education Assistance Act. Applicants serving multiple tribes must submit letters of support and tribal resolutions from each tribe to be served. Urban Indian organizations must provide proof of nonprofit status and demonstrate governance by an urban Indian-controlled board.

The cooperative agreement funds a culturally responsive PHN model emphasizing preventive health and wellness services, care access expansion, early identification and management of health conditions, and support for healthy lifestyle behaviors through coordinated, community-based interventions. The program supports screening, home visits, immunizations, maternal-child health care, elder care, chronic disease management, school-based services, health promotion, disease prevention, case management, population-based services, and community disease surveillance. Projects may support transitions of care from clinical settings into community settings.

Project periods extend up to five years. After the initial competitive award, continuation applications are submitted annually on a noncompetitive basis. Selection criteria include statement of problem, need for assistance, expected results or benefits, soundness of approach and project plan, key personnel capability, adequacy of management controls, and compatibility with Indian Health Service goals and objectives.

The program operates under CFDA 93.383. Recent competitive awards in September 2022 ranged from $1.225 million to $1.675 million each.
