About this opportunity

The Department of Health and Human Services Indian Health Service is soliciting applications for cooperative agreements to fund produce prescription pilot programs in Tribal communities, with applications due by 11:59 p.m. Eastern Time on June 22, 2026. Only Federally recognized Indian Tribes, Tribal organizations, and Urban Indian organizations currently administering contracts under 25 U.S.C. 1653 are eligible to apply. There is no cost-sharing requirement.

The program will award $3.5 million across 14–18 cooperative agreements, providing $200,000 to $250,000 per applicant for the first budget period. The period of performance spans five years in five 1-year budget periods. Work is performed within Tribal communities and through partnerships with health care providers and food suppliers.

Awardees must develop produce prescription program infrastructure, identify eligible American Indian and Alaska Native populations, establish partnerships with health care providers and food suppliers, implement community-appropriate nutrition education, and collect and report baseline and follow-up data on food insecurity, nutrition education, and prescription distribution and redemption. Applicants must maintain a long-term sustainability plan beyond federal funding. All awardees must attend quarterly conference calls, submit annual progress reports and data, budget for at least two staff to attend a two-day in-person workshop, participate in training and site visits, and designate an officially approved Project Director with relevant health or wellness education, award program management experience, and working knowledge of nutrition challenges in American Indian and Alaska Native communities.

Applicants must submit a draft or final Tribal Resolution, proof of nonprofit status (for Urban Indian organizations), biographical sketches for key personnel, letters of support, indirect cost agreements if applicable, and audit documentation, and must not have any missing or delinquent audits. The expected award date is July 15, 2026, with an earliest start date of July 15, 2026. This opportunity is funded under CFDA 93.933, Demonstration Projects for Indian Health.

Synopsis

This program supports Tribes, Tribal organizations, and Urban Indian organizations in implementing produce prescription models that increase access to nutritious and community relevant foods and promote whole-person health and well-being.
