About this opportunity

The Department of Health and Human Services' Administration for Children and Families (ACF) is seeking applicants for the Fiscal Year 2025 Transitional Living Program (TLP), which provides comprehensive services to homeless youth ages 16-21. The program aims to support youth through street-based outreach, safe shelter, and services designed to help participants achieve self-sufficiency, focusing on four core outcome areas: safe housing, education/employment, permanent connections, and social-emotional well-being. Eligible applicants include public and nonprofit entities, state and local governments, educational institutions, and Native American tribal organizations. Evaluation criteria will include the applicant's ability to provide trauma-informed care, develop comprehensive services, and demonstrate potential for helping youth transition to independent living. The application is due by July 28, 2025, with an expected project start date of September 30, 2025.

The grant opportunity anticipates approximately 67 awards, with individual project funding ranging from $100,000 to $350,000, and a total program funding of $23,434,303. Grantees must provide a 10% cost-sharing match and are expected to offer intensive services including screening, case management, life skills training, transition planning, and aftercare support. The project period is three years, with potential non-competing continuation awards possible for the second and third budget periods based on satisfactory progress. Successful applicants will be required to submit semi-annual performance and financial reports and participate in training and technical assistance provided by the Runaway and Homeless Youth Training, Technical Assistance, and Capacity Building Center.

Synopsis

The Transitional Living Program (TLP) provides street-based outreach, shelter and comprehensive supportive services to youth ages 16 through 21 for up to 18 months or, under extenuating circumstances, 21 months. In addition to shelter, TLPs provide comprehensive services that support participating youth"s transition to self-sufficiency and stable, independent living. Through the combination of shelter and services, TLP youth are expected to show improvements in four core outcome areas: safe and stable housing, education or employment, permanent connections, and social and emotional well-being.
