About this opportunity

The Health Resources and Services Administration (HRSA) is soliciting applications for the Fiscal Year 2027 Service Area Competition (SAC) to fund primary health care delivery in medically underserved communities across the United States and its territories, with service areas announced in the Service Area Announcement Table (SAAT).

Eligible applicants include public or Indian housing authorities, state and county governments, special district governments, federally recognized and non-federally recognized tribal governments and organizations, city and township governments, and 501(c)(3) nonprofits. HRSA funds comprehensive preventive and primary health care services, including medical, dental, behavioral health, and pharmacy care, through grants supporting community health centers (CHC), migratory and seasonal agricultural workers (MSAW), homeless populations (HP), and residents of public housing (RPH). Awards may also support new health centers, service delivery site expansion, innovative evidence-based models, and administrative priorities addressing emerging public health issues. Applicants must demonstrate compliance with Health Center Program requirements under Section 330(e) of the Public Health Service Act.

Awards operate under CFDA 93.224, the Health Center Program. Recent awards in the portfolio range from approximately $544,000 to $44 million, with typical awards in the low single-digit millions. Continued funding beyond the first budget period depends on congressional appropriation, statutory and regulatory compliance, satisfactory progress toward project objectives, and the government's determination that funding remains in its best interest.

Place of performance encompasses the 50 states, District of Columbia, Puerto Rico, Northern Mariana Islands, American Samoa, Guam, U.S. Virgin Islands, Federated States of Micronesia, Republic of the Marshall Islands, and Republic of Palau. Preapplication coordination is required, and environmental impact information may be required for capital projects. Recipients expending $1 million or more annually in federal awards must undergo single or program-specific audits; grant accounting records must be maintained for three years after Federal Financial Report submission.

Forecast synopsis

The FY 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. For each SAC Notice of Funding Opportunity (NOFO), service areas available for competition are announced in the Service Area Announcement Table (SAAT). Throughout the year, we may announce additional service areas in a Service Area Competition-Additional Area (SAC-AA) if an awarded health center cannot continue to provide services in the funded service area.
