About this opportunity

The Health Resources and Services Administration (HRSA) is soliciting applications for the Fiscal Year 2027 Health Center Program Service Area Competition (SAC), a forecasted grant opportunity to fund comprehensive primary and preventive health care delivery in medically underserved communities and populations across the United States and territories. Service areas available for competition are announced in the Service Area Announcement Table (SAAT); additional service areas may be announced throughout the year as SAC-Additional Area (SAC-AA) if incumbent health centers cannot continue service.

Eligible applicants include domestic private nonprofits, public agencies, faith-based organizations, tribes, tribal organizations (federally and non-federally recognized), city and county governments, state governments, special district governments, public or Indian housing authorities, and 501(c)(3) and non-501(c)(3) nonprofits. HRSA funds health centers under Section 330(e) of the Public Health Service Act to deliver primary care to medically underserved communities and special populations—migratory and seasonal agricultural workers (MSAW funding), homeless populations (HP funding), and residents of public housing (RPH funding). Funding may also support new health centers, expanded service delivery sites, administrative priorities, emerging public health issues, and innovative, evidence-based models to increase access to primary health care.

Recent Health Center Program awards range from approximately $544,000 to $43.95 million, with typical awards in the low single-digit millions. Awards support expanded access hours (evening, weekend, and early-morning availability), addressing barriers for working families and vulnerable populations. Continuation awards beyond the first budget period are contingent on congressional appropriation, compliance with statutory and regulatory requirements, satisfactory progress toward project objectives, and a finding that continued funding serves the government's best interest. Preapplication coordination and intergovernmental review (E.O. 12372) are required. HRSA operates under CFDA 93.224.

Forecast synopsis

The FY 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. For each SAC Notice of Funding Opportunity (NOFO), service areas available for competition are announced in the Service Area Announcement Table (SAAT). Throughout the year, we may announce additional service areas in a Service Area Competition-Additional Area (SAC-AA) if an awarded health center cannot continue to provide services in the funded service area.
