About this opportunity

The Health Resources and Services Administration (HRSA) is forecasting FY 2027 Service Area Competition (SAC) grants to support delivery of comprehensive primary health care services in medically underserved communities and populations across the United States and its territories. Service areas available for competition will be announced in the Service Area Announcement Table (SAAT) accompanying each SAC Notice of Funding Opportunity; additional service areas may be announced throughout the year if awarded health centers cannot continue operations.

Eligible applicants include federally recognized tribal governments, city and township governments, special district governments, public or Indian housing authorities, non-federally recognized tribal organizations, 501(c)(3) nonprofits, state governments, county governments, and non-501(c)(3) nonprofits.

Awards support primary care service delivery to medically underserved communities (community health center funding) or to special medically underserved populations including migratory and seasonal agricultural workers, homeless individuals, and residents of public housing. Funding may support new health centers and service delivery sites, innovative evidence-based models to expand access to primary health care, and activities addressing administrative priorities or emerging public health issues. Awards may also support expanded hours, dental services, behavioral health, and pharmacy services.

The program operates under CFDA 93.224 (Health Center Program). Recipients expending $1 million or more annually in federal awards must undergo single or program-specific audits. Recipients must maintain accounting records for three years after submitting the Federal Financial Report. Preapplication coordination and intergovernmental review under E.O. 12372 are required. Continued funding beyond the first budget period is contingent on congressional appropriation, compliance with statutory and regulatory requirements, satisfactory progress toward project objectives, and a government determination that continued funding serves the public interest.

Forecast synopsis

The FY 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. For each SAC Notice of Funding Opportunity (NOFO), service areas available for competition are announced in the Service Area Announcement Table (SAAT). Throughout the year, we may announce additional service areas in a Service Area Competition-Additional Area (SAC-AA) if an awarded health center cannot continue to provide services in the funded service area.
