About this opportunity

The Health Resources and Services Administration is soliciting applications for the Fiscal Year 2027 Health Center Program Service Area Competition to fund delivery of comprehensive primary and preventive health care services to medically underserved populations, with Phase 1 applications due October 19, 2026, at 11:59 p.m. Eastern Time. Applicants must project serving at least 75 percent of the SAAT Patient Target to be eligible, with funding reductions applied for projections below 95 percent of the target.

Eligible applicants include domestic public agencies, Native American tribal governments and organizations, nonprofits including 501(c)(3) and non-501(c)(3) entities, public housing authorities, and tribal organizations not federally recognized, provided they serve medically underserved areas or populations within announced service areas. Applicants must provide all required Health Center Program primary and preventive services on a sliding fee basis, operate at least one full-time service delivery site offering 40 or more hours weekly, directly employ the Chief Executive Officer, and maintain substantive organizational control.

HRSA anticipates approximately $267.8 million in total available funding distributed across approximately 63 competitive grants, with awards offering either one-year or four-year periods of performance. Year one funding requests cannot exceed amounts listed in the Service Area Announcement Table. New applicants and competing supplement applicants must open all proposed sites within 120 days of award and achieve full operational status within one year.

Merit review evaluates applications across eight criteria totaling 100 points: Need (10), Response (15), Collaboration (10), Impact (15), Capacity (15), Governance (10), Continuity of Care (15), and Support Requested (10). Three funding priorities offer bonus points for positive patient trends, Patient-Centered Medical Home recognition, and organizations not currently holding Health Center Program awards. Phase 2 supplemental information through HRSA Electronic Handbooks is due November 18, 2026, at 5 p.m. Eastern Time, with expected award date April 1, 2027. This opportunity falls under CFDA 93.224.

Synopsis

The FY 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. For each SAC Notice of Funding Opportunity (NOFO), service areas available for competition are announced in the Service Area Announcement Table (SAAT). Throughout the year, we may announce additional service areas in a Service Area Competition-Additional Area (SAC-AA) if an awarded health center cannot continue to provide services in the funded service area.
