Eligible applicants include higher education institutions (public and private), county and state governments, city and township governments, special district governments, independent school districts, public or Indian housing authorities, tribal governments (federally recognized and other), 501(c)(3) nonprofits, non-501(c)(3) nonprofits, for-profit organizations, small businesses, and other entities. Additional eligible applicants include Alaska Native and Native Hawaiian Serving Institutions, Asian American Native American Pacific Islander Serving Institutions, eligible federal agencies, faith-based or community-based organizations, Hispanic-serving institutions, Historically Black Colleges and Universities, tribally controlled colleges and universities, U.S. territories or possessions, regional organizations, and non-domestic entities.

The opportunity operates under CFDA numbers 93.393, 93.394, 93.395, 93.396, and 93.399, spanning cancer biology, detection and diagnosis, treatment, cause and prevention, and control research programs. The R21 mechanism is clinical trial optional, supporting early-stage exploratory work. Applications undergo scientific peer review assessing scientific merit and significance, technical adequacy of experimental design and approach, investigator competency, adequacy of facilities and resources, budget necessity relative to proposed work, and relevance to program objectives. All competitive applications compete for available funds based on scientific and technical merit, program relevance, and program balance consistent with NCI and NIH priorities and strategic plans.
