About this opportunity

The Department of Health and Human Services Substance Abuse and Mental Health Services Administration (SAMHSA) is soliciting applications for Assisted Outpatient Treatment (AOT) program implementation funding; the April 20, 2026 application deadline applies to solicitation SM-26-001. Eligibility is statutorily limited to states, counties, cities, mental health systems (including state mental health authorities), mental health courts, or any other entities with authority under applicable state law to implement, monitor, and oversee AOT programs. The funding instrument is a Cooperative Agreement, marking a shift from prior Project Grant awards.

Projects are funded for up to four years, with electronic fund transfer payments. SAMHSA aims to reduce psychiatric hospitalization, homelessness, incarceration, and criminal justice involvement while improving health and social outcomes for adults with serious mental illness. Recipients implement multidisciplinary AOT teams working with civil courts, community partners, hospitals, and crisis services; funded activities include intensive case management, medication management, psychiatric care, peer support, crisis intervention, supported housing, supported employment, and wraparound social services.

Applications are scored on merit and peer review criteria and generally funded in score order. The Assistance Listing number is 93.997. Recent award velocity demonstrates program growth: 22 grants were obligated in 2024 alone, compared to three or fewer in prior years, with recent awards ranging from $5.7 million to $8.6 million across four-year periods. The portfolio reflects $262.5 million in federal obligations and $20.8 million in non-federal cost-share across 83 awards since 2016. Applicants must comply with 2 CFR 200 Uniform Administrative Requirements; a three-year record retention requirement applies, extended if final audit remains pending.

Forecast synopsis

The purpose of this program is to facilitate the implementation of Assisted Outpatient Treatment (AOT) for adults with serious mental illness (SMI). Recipients are expected to implement the AOT program, working with civil courts, community partners, and other entities to support community-based treatment for adults with SMI who meet criteria for AOT.

Eligibility

Eligibility is statutorily limited to counties, cities, mental health systems (including mental health authorities), mental health courts, or any other entities with authority under the law of the State in which the applicant is located to implement, monitor and oversee assisted outpatient programs.
