About this opportunity
The Substance Abuse and Mental Health Services Administration (SAMHSA) is soliciting applications for the Assisted Outpatient Treatment (AOT) Program, a cooperative agreement opportunity designed to implement community-based treatment for adults with serious mental illness. The program requires recipients to work collaboratively with civil courts, community partners, and other entities to establish AOT services for individuals who meet state-specific criteria for assisted outpatient treatment. Eligible applicants include states, counties, cities mental health systems, state mental health authorities, mental health courts, and other entities with legal authority to implement and oversee AOT programs. SAMHSA will evaluate applications based on merit review and peer review criteria, with applications generally funded in score order. The proposal deadline is April 20, 2026, with the opportunity posted on December 10, 2025.
Federal grant awards under this program typically range from approximately $1 million to $8.7 million, with projects funded for up to four years. The implementation period extends across multiple years, allowing recipients to develop and sustain comprehensive treatment programs that include case management, psychiatric care, medication management, and community-based rehabilitation services. Performance and payments will be managed through an Electronic Transfer System. Recipients must maintain records for a minimum of three years, with extended retention required until final audits are completed and findings resolved, in accordance with 2 CFR 200 requirements. Performance will occur in the applicant's local community and state jurisdiction where the AOT program is implemented.
