About this opportunity
The Administration for Community Living is soliciting cooperative agreement applications to develop and expand dementia-capable community health worker programs within the National Aging Network, with an application due date of approximately November 24, 2026.
Applicants must be an Area Agency on Aging or demonstrate active participation in the local aging services network and partnership with an AAA in the project service area. Faith-based and community organizations that meet eligibility requirements are eligible; foreign entities and individuals are not. Community program applicants may not hold more than one ADPI grant at any time.
Awards are cooperative agreements with typical durations of 36 months. Support beyond year one is contingent on satisfactory progress and fund availability. Recipients obligated $750,000 or more annually in federal awards must undergo single or program-specific audit; records must be retained for three years after final expenditure report submission.
Projects will integrate community health workers into community-based aging services to improve dementia screening and referral, deliver dementia-specific evidence-based or evidence-informed interventions, reduce barriers to care, and improve access to supportive services and quality of life for people living with dementia and their caregivers. The program emphasizes rural and underserved communities.
The Administration for Community Living funds under CFDA 93.470. Assistance is released by letter of credit. Applications are evaluated competitively by field experts; specific evaluation criteria will be detailed in the final funding opportunity announcement. Historical award patterns through the Alzheimer's Disease Program Initiative cluster near $750,000 to $1 million for three-year terms.
