About this opportunity

The Administration for Community Living (ACL) is seeking twelve cooperative agreements to develop and expand dementia-capable Community Health Worker (CHW) programs within the National Aging Network through the Alzheimer's Disease Programs Initiative (ADPI). The primary objective is to enhance workforce capacity by training CHWs to support people living with dementia and their caregivers, with a focus on improving health conditions in communities with high disease, disability, and mortality rates. Key requirements include implementing a dementia-specific evidence-based intervention, progressively allocating increasing percentages of the budget to direct services (25% in year 1, 35% in year 2, and 45% in year 3), and meeting a 25% cost-sharing requirement. Applicants will be evaluated based on their ability to develop comprehensive CHW training plans, implement community-based dementia screening, and address barriers to care. Critical dates include an informational conference call on July 9, 2025, and the application deadline of July 30, 2025.

Each cooperative agreement will provide up to $750,000 in federal funding over a 36-month period, with the anticipated project start date of September 1, 2025. The program will target Area Agencies on Aging and states designated as single planning and service areas, emphasizing the development of a culturally competent dementia CHW workforce. The initiative aims to support rural and underserved communities by creating a career advancement pathway for CHWs and increasing access to meaningful, culturally relevant information and services for family caregivers. The place of performance will be within the selected organizations' service areas, with a focus on developing and implementing dementia-specific programs that expand the traditional role of Community Health Workers.

Synopsis

Through the Alzheimer"s Disease Program Initiative (ADPI), ACL seeks to enhance workforce capacity in the National Aging Network with the inclusion of dementia-capable Community Health Workers (CHWs). CHWs work to improve conditions for health in communities with the highest rates of disease, disability, and death. ADPI supports dementia-capable home and community-based services for people living with dementia and their caregivers. As a result of this initiative, ACL seeks to strengthen relationships with rural and underserved communities and address barriers to care for people living with dementia and their caregivers.
