About this opportunity
The Indian Health Service (IHS), under the Department of Health and Human Services, is soliciting cooperative agreement applications to support the enhancement of dementia care services in Tribal and Urban Indian communities. Applicants must demonstrate existing infrastructure in at least two of five primary care drivers (awareness, diagnosis, assessment, management, and caregiver support) and provide ambulatory care services. The program aims to expand local capacity for culturally relevant comprehensive care, enhance clinical services, develop long-term sustainability plans, and document emerging care models. Evaluation criteria include the statement of problems requiring solution, applicant need for assistance, expected results and benefits, soundness of approach and project plan, key personnel capability, and adequacy of management controls. The application deadline is August 1, 2025, with an expected award date of September 1, 2025.
This three-year cooperative agreement program has an expected total funding of $1.6 million distributed across approximately 8 awards, with individual awards ranging from $100,000 to $200,000 per applicant for the first budget period. Eligible applicants include federally recognized Indian Tribes, Tribal organizations, and Urban Indian Organizations. Performance will take place within applicant organizations serving American Indian and Alaska Native populations, with successful awardees expected to develop and implement dementia care tools and resources while establishing sustainable program models within their respective communities.
