Quotes must include an executed SF 1449 form with the required VAAR 852.219-75 certification, detailed pricing for all line items, a technical approach demonstrating required qualifications, and relevant past performance documentation. Services will be performed at the Seattle VA Medical Center (1660 S Columbian Way, Seattle, WA 98108) and American Lake VA Medical Center (9600 Veterans Drive SW, Tacoma, WA 98493), with payment via Electronic Funds Transfer upon completion and acceptance of work.
