About this opportunity

The Department of Veterans Affairs, Veterans Integrated Service Network 21, Network Contracting Office (NCO 21) intends to sole source a Fixed Price Indefinite Delivery Indefinite Quantity (FP-IDQ) contract to Care Meridian (dba NeuroRestorative) for community nursing home services, with a response deadline of July 14, 2026 at 12:00 PM Pacific Time for capability statements.

Performance will occur in Reno, Nevada. The contract covers continued care for seven Veterans currently receiving services at the facility. The period of performance runs from October 24, 2026 through October 23, 2031.

The VA's sole source rationale centers on continuity of care and Veteran choice. The Community Nursing Home Program prioritizes enabling Veterans to select facilities near their homes and families while maintaining local oversight. The VA states it does not intend to move the Veterans or disrupt the services currently provided by Care Meridian due to the critical nature of care continuity at this location.

All responsible sources may submit capability statements to Contracting Officer Emily Trinh (Emily.Trinh@va.gov) by the July 14 deadline. The VA will consider submissions before determining whether to proceed with the sole source acquisition. The solicitation number is 36C26126Q0897 (NAICS 623110, PSC Q402).

PRE-SOLICITATION NOTICE OF INTENT TO SOLE SOURCE

Department of Veterans Affairs (VA), Network Contracting Office (NCO 21), intends to award a sole source, Fixed Price Indefinite Delivery Indefinite Quantity (FP-IDQ) contract to Care Meridian (dba NeuroRestorative) (UEI: J4BLAP1ET1D5). The Contractor is currently providing community nursing home services to seven (7) Veterans at the facility and a replacement contract is being established to provide continued care for the Veterans at the CNH facility.

All responsible sources may submit a capability statement to Emily.Trinh@va.gov, which will be considered by the agency on or before July 14, 2026 at 12:00PM PST. After consideration, the Government will determine whether to proceed with the sole source acquisition. Please keep in mind that patient care and choice in the decision is the highest priority to the VA. As prescribed in VHA Notice 2020-32 and VHA Handbook 1143.2, for more than 35 years the CNH Program has maintained two cornerstones: some level of patient choice in choosing a nursing home close to the Veteran s home and family, and a unique approach to local oversight of CNHs. In light of this program, the VA does not intend to move the Veterans or disrupt the much-needed services, in particular locale, that is currently being provided by Care Meridian (dba NeuroRestorative)

Period of Performance: 10/24/2026 10/23/2031
