The current effort is a modification to Task Order (TO) FA8052-18-F-0050, Zero Suicide - Mental Health Case
Managers, awarded against Defense Health Agency (DHA) Medical Q-Coded Services (MQS) basic multiple
award contract number HT0050-18-D-0032. The contract line items are firm fixed-price. This justification is
to modify the existing TO by extending two Full Time Equivalent (FTE) personnel one each to be located at
Little Rock AFB, AR and Whiteman AFB, MO. This FOE amends a previous FOE <SAT, and IAW FAR 16.505(b)(2)(ii)(D) did not require posting. Given that this amended FOE exceeds the SAT, the requirement for posting is now required.
Case managers coordinate the treatment of behavioral health conditions (mental health and substance use)
and physical health conditions by providing comprehensive care management, care coordination, transitional
care, and health promotion to improve overall wellness and manage chronic health conditions. Key case
management activities include coordinating care with multiple providers across all levels and sites of care,
addressing behavioral and medical needs of patients and their families/caregivers. Case managers
participate in multidisciplinary patient care management practices, command consultation, evaluating care
and outcomes to ensure timely and appropriate provision of services, safety assessment and safety planning.
