About this opportunity

The Centers for Medicare and Medicaid Services is issuing a Request for Information to gather stakeholder feedback on the development of IT standards for Medicaid Enterprise Systems, with responses due September 25, 2026. This is an information-gathering exercise only; CMS is not soliciting proposals, will not reimburse response costs, and does not commit to issuing a future Request for Proposal or contracting vehicle.

CMS seeks input from State Medicaid Agencies, MES technology vendors, systems integrators, Standards Development Organizations, healthcare providers, and industry experts to inform modernization of Medicaid Enterprise Systems through standards-based, commercial-off-the-shelf modular solutions and cloud-based technologies. The initiative aims to improve interoperability, reduce vendor lock-in, and lower duplicative costs across state implementations, guided by federal regulations at 42 CFR Parts 433 and 95.

The RFI comprises 40 targeted questions organized by stakeholder category. State Medicaid Agencies are queried on technical and operational barriers to interoperability and modularity, underutilized industry standards such as HL7 FHIR and NIST frameworks, workforce skill gaps, data governance challenges, and data elements lacking standardization. Vendors and systems integrators are questioned on COTS alignment with varying state requirements, standards engagement processes, vendor lock-in mitigation, cloud security and FedRAMP considerations, and implementation complexity. Standards Development Organizations and industry experts are asked to identify emerging standards addressing identity management and data quality, recommend governance models, harmonize MES standards with broader HHS and Office of the National Coordinator interoperability rules, and address privacy standards and technical debt prevention.

Respondents must preface answers with corresponding question labels (such as SA-1, V-2) and submit responses via email to MACBIS-Enterprise-Services@cms.hhs.gov and aaron.blackshire@cms.hhs.gov.

Update #2 · Latest ·

CMS seeks feedback from all Medicaid Enterprise System (MES) stakeholders to inform the development and implementation of the MES IT Standards Program. We invite input from State Medicaid Agencies, MES technology vendors, systems integrators, Standards Development Organizations (SDOs), healthcare providers, and other industry experts.

The goal of this RFI is to gather specific, actionable information to ensure the program effectively addresses the most significant barriers to MES transformation. We seek to understand which elements of the current ecosystem are working well, which require improvement, and how CMS can best support states and vendors in transitioning to a more standardized, interoperable, and cost-effective future.

To aid in our understanding of the submitted responses, please be clear and concise and preface your responses with the corresponding question label (e.g., SA-1, V-2). The RFI will be open for 45 days from the date of posting. Please submit responses via email to MACBIS-Enterprise-Services@cms.hhs.gov and aaron.blackshire@cms.hhs.gov.
