Update #4 · Latest ·
a. The purpose of this amendment is to incorporate additional Questions & Answers (Q&As) that were received for this solicitation:
Q1. How many pharmacy technicians need to complete the gloved-tip sampling? This service is rated per person. Please confirm that TASK 002 is based on a total of twelve (12) employees per contracted year. I could not find the quantity of technicians/employees in paragraph 3.1.1, 3.1.2, and 3.1.3 of the Performance of Work.
A1. There is a total of 10 radiopharmaceutical technicians. The sampling will be performed once a year to each technician. Half the technicians will be tested at six months and the other half six months thereafter. New technicians will be sampled 3 times initially. As a result of this clarification, the quantities for Line Items 0002, 1002, 2002, 3002, and 4002 are increased from 12 to 20 each. The quantities for Line Item 5002 is increased to 10. Contractors are reminded that the resulting contract will be a Firm-Fixed Price Requirements Type contract and that the government will issue orders against it.
Q2. What is the risk type for the media fill kits?
A2. Media fill kits are low risk.
Q3. Referencing Section 3.1.4.4 in the PWS, how many rooms have the low air returns?
A3. Two rooms have low air returns.
Q4. Can the Asbestos clause in the contract be omitted?Â
A4. According to the Government records, Asbestos Containing Building Material (ACBM) was removed from rooms A13, A13a, and A13b during a recent project renovation. Work by contractor will not be expected to be in areas where ACBM is present.
Q5. Is this bid just for the facility in San Juan PR or if there are other locations that may be included?
A5. Work will be performed in San Juan facility only.
b. The solicitation set-aside is corrected to read: Service-Disabled Veteran-Owned Small Business. The previous amendment left this area blank and is corrected with this amendment.
c. The solicitation closing date and time remain unchanged at March 26, 2021 at 2:00 PM (EST).
d. All other terms and conditions remain unchanged. Contractors are reminded to acknowledge this amendment and include a copy of it in the quote package.

CONTINUATION PAGE
a. The purpose of this amendment is to incorporate the following Questions & Answers related to this solicitation:
Q1. Can we obtain a previous report for the facility? Can we get a description of the room and the Nuclear Medicine Service area? (for example, 3 room suite ante, IV sterile, HD sterile, 1 bsc, 1 lfh, 6 ceiling HEPA filters, Sample locations, how are the HEPA filters challenged?)
A1. See documents HSS Report PR VA USP 797 Compliance-Nuclear Medicine 20200619 and 5002-221689-21506 , which provides the requested information.
Q2. For these three Items
3.1.1 Gowning and Garbing Evaluation
3.1.2 Gloved fingertip and thumb sampling
3.1.3 Media-Fill Testing
Will all employees be available on the same day and on the same shift for this evaluation? If no, how many days and shift would be expected?
A2. The Government will coordinate with the service to have all the employees available during scheduled visits.
Q3. Performance summary. This matrix is not very clear; PRS#1, 2 and 3Â and under remedies column it says, "Re-perform tasks within 3 working days at no additional cost to the Government". Can you provide further explanation?
A3. The remedies under the PRS are provided to the contractor as an opportunity to correct defective services, which are not as per the Performance Work Statement. An example may be that the service was schedule for X date and the contractor failed to arrive in such date. Another example maybe that the contractor did not test all the equipment and/or areas during the visits, etc. In these cases, the contractor will have to re-perform the services within three calendar days without additional cost to the government.
b. The closing date and time of this RFQ is extended through March 23, 2021 at 2:00 PM. Contractors are reminded to acknowledge this amendment and include a copy of it in the quote package.
c. All other terms and conditions remain unchanged.
