AMMENDMENT 1 - amendment refers to page 6, paragraph 6, VA San Diego Healthcare System, Biweekly Requirements, subparagraph 3.
Currently reads: "Treat all fixture drains (approximately 50) with Enzyme 80 (blend of drain bacteria) or its equivalent; manufacturer recommendations are to be followed."
Amended to: "Treat all 50 fixture drains (30 fixtures at Canteen Kitchen, Building 1, 1st floor and 20 fixtures at Dietetic Kitchen, Building 1, Basement) with Enzyme 80 (blend of drain bacteria) or its equivalent; manufacturer recommendations are to be followed."
AMMENDMENT 2 - addition of a third requirement to page 6, paragraph 6, VA San Diego Healthcare System, Monthly Requirements. It is to read as follows: "Fill the 55 gallon drum with enzyme 80 or its equivalent. The 55 gallon drum is located at the Dietetic Kitchen, Building 1."

VENDOR QUESTIONS AND ANSWERS
1. Will a payment and performance bond be required for the bid proposal or will a letter of bond elegibility from our bonding company be sufficient to submit with the offer?
No bond is required for the execution of this service and offeror is not required to submit a Letter of bond eligibility with their offer.
2. Please verify quantities of all the drains requested to be included in the solicitation from each location.
There are 50 drain fixtures total at VA San Diego Healthcare System; 30 fixtures at Canteen Kitchen, Building 1, 1st floor; and 20 fixtures at Dietetic Kitchen, Building 1. There are 10 drain fixtures located at the main kitchen area at the VA Aspire Center.
3. Please clarify if the patient treatment rooms are to be included.
Patient treatment rooms are not part of this requirement.
4. Please clarify if the service includes the use of a 55 gallon drum to pump enzyme to the grease trap interceptor. Maintain enzyme level up to the 55 gallon on monthly basis.

Combined Synopsis and Solicitation
Department of Veterans Affairs, Network Contracting Office (NCO 22), 4811 Airport Plaza Drive, Suite 600, Long Beach, CA, 90815 as directed by VA San Diego Healthcare Systems (VAGLAHS) has a requirement for annual Grease Trap and Drainage Preventive Maintenance Services. The Government intends to fulfil this requirement with a base and four option year firm fixed price contract. This procurement is an open market small business total set aside under FAR 13 Simplified Acquisition Procedures.
The Contractor shall have the ability to provide all necessary labor, materials, supplies, tools, equipment, transportation, and supervision to perform grease trap and drainage preventive maintenance functions at VA San Diego Healthcare System (VASDHS), 3350 La Jolla Village Drive, San Diego, CA, 92161 and VA Aspire Center, 2121 San Diego Ave, San Diego, CA 92110.
Offerors are highly encouraged to attend a Site Visit scheduled on September 4, 2014 at 10:30 a.m. Pacific Standard Time (PST) at VAGLAHS, 3350 La Jolla Village Drive, San Diego, CA. The interested parties will immediately proceed to VA Aspire Center, 2121 San Diego Ave, San Diego, CA 92110 after the VAGLAHS visit. The offerors are responsible for providing their own transportation from site to site. The site visit is anticipated to last 3 hours. The Contractor must RSVP at edgar.alvizar@va.gov no later than September 2, 3:00 p.m. PST.
Offers to this combined synopsis and solicitation shall be e-mailed not later than 11:00 a.m. PST on September 11, 2014 to the attention of Edgar Alvizar at edgar.alvizar@va.gov with "Interested Contractor - Grease Drainage Preventive Services" on the subject line. Files should be sent in PDF format and no larger than 10MB or it may be rejected by the server. Vendor shall expect a confirmation e-mail indicating offer was successfully received by the Government. The Government is not responsible for rejected e-mails that exceed the e-mail capacity. Telephone responses shall NOT be accepted.
Offerors shall submit their offers in accordance with the attached solicitation (see E.2 ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS and E.2 ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS).
