Question: Is there is a standard form(s) you would have us use or reference for the submission?

Answer: There is no standard form. Just a brief description of how you will complete the work, and then a cost breakout by labor categories, number of labor hours, and indirect costs. And also a copy of your indirect rate agreement if you have one.
