About this opportunity
The Indiana Army National Guard seeks dental surgical services for approximately 10,500 soldiers, with proposals due September 11. The procurement is set aside for Total Small Business and structured as an unpriced purchase order under FAR 12.201-1(e)(2) with a not-to-exceed amount of $100,000 on a firm fixed-price basis.
Place of performance is Edinburgh, Indiana, within a 50-mile radius of Camp Atterbury. The contractor must provide surgical extractions, crowns, root canals, core build-ups, and associated procedures from its own dental office during normal business hours. Contractors must accept Concordia and Blue Shield Blue Cross insurance; soldiers' existing coverage will be billed first, with any remaining balance invoiced to the government through the Wide Area Workflow system.
The period of performance runs from September 21, 2026 to September 20, 2027. Treatment requests arrive via Standard Form 603 through DODSafe identifying soldiers designated Dental Readiness Code (DRC) 3, non-deployable personnel requiring conversion to DRC2 status. The contractor must schedule patients within 14 calendar days of receiving a treatment request and submit monthly invoices by the last business day of each month, accompanied by completed DD Forms 2813 showing updated DRC codes. The Performance Work Statement projects approximately 300 dental treatments for the fiscal year.
Contractors must demonstrate prior experience with government contracts and unpriced purchase order payment arrangements, provide the exact office location where services will be delivered, and submit retail price sheets or fee schedules. The government will evaluate performance through a Quality Assurance Surveillance Plan with acceptable quality levels of less than 2% defect rates for scheduling compliance, monthly billing submission, and DD Form 2813 submission. The NAICS code is 621210, Offices of Dentists; the PSC is Q503, Medical–Dentistry. The contracting officer is Thomas Fleming, 520-713-6849.
