Location of services should be within a 10 miles radius of the Lane County Department of Public Safety RRC/Lane County Work Release at 75 West 5th Avenue, Eugene, Oregon 97401 . Performance periods under this contract will be for a one-year base period to begin on October 01, 2015 with four (4) one-year option renewal periods with service ending (if all options are renewed) on September 30, 2020. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor.

Substance Abuse Treatment Services:

Intake Assessments & Report (one per authorized inmate) for 15 inmates = 15 assessments

Individual Counseling Sessions (30 minutes = 1 unit) 48 units (per inmate) x 15 = 720 units

The services shall be in accordance with the Federal Bureau of Prisons Community Treatment Services most recent Statement of Work dated April 2011- No Masters Required, Revision 01, 2013, PSB. The request for quote will be available on or after June 19, 2015 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the request for quote will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site.
